How Much Should Physician Offices Spend on Paid Media? A Digital Marketing Strategy for Healthcare

A question ARSNL often hears is: “What percentage of our marketing budget should go to paid media?”

‍

Here’s the short answer: there isn’t one answer.

‍

For physician offices, an effective digital marketing strategy for healthcare needs to account for more than a standard percentage of revenue or a predetermined advertising budget. The right paid-media investment depends on your practice’s utilization, capacity, patient acquisition channels, and the specific business problem you are trying to solve. A percentage should not come first. It should be the result of a practical model built around your practice’s economics.

‍

Before getting into media strategy, start with a more important question: How well are you using the capacity you already have?

‍

If physicians are fully booked, referral networks are strong, organic channels are consistently generating new patients, and demand already exceeds capacity, incremental paid media may not be necessary. However, if physicians have open schedules, new providers need to build their patient panels, or a practice is introducing a service patients do not yet know to ask for, paid media may have a meaningful role to play.

Start With the Utilization Budgeting Model

At ARSNL, we begin by identifying the gap between current and desired utilization.

‍

  • What percentage increase in utilization is realistic for this practice?
  • How many additional consultations, procedures, or surgeries are needed to reach that goal?
  • Are there operational constraints, such as provider availability, scheduling, staffing, or operating-room capacity, that limit growth?
  • Which physicians, locations, or service lines have the greatest opportunity for incremental volume?

‍

A goal such as increasing consultations by 15% is only useful if the practice has the capacity to deliver that care and the current patient-acquisition funnel can realistically support it.

Model It Out

Once the utilization target is clear, work backward from the number of additional patients or procedures needed.

‍

The process looks like this:

‍

  1. Define the utilization gap.
Determine the difference between current volume and the desired level of utilization.
  2. Calculate the additional patient volume needed.
If a practice needs fifteen more surgeries per month to reach its target, that is the starting point, not a predetermined media percentage.
  3. Apply the current lead-to-patient conversion rate.
If 10% of qualified leads become patients, the practice needs approximately 150 additional qualified leads to generate fifteen additional patients.
  4. Use the current cost per lead.
If the practice’s cost per qualified lead is $100, generating those 150 leads would require an estimated $15,000 in media investment.
  5. Compare the investment with patient value.
Evaluate the projected cost against the average value of a new patient, procedure, or patient engagement. This helps determine whether the investment supports a sustainable return, not simply more activity.

‍

This is how a paid-media budget becomes an informed business decision rather than an arbitrary percentage.

Understand What Paid Media Can Solve

Before allocating budget, ARSNL also looks at where patients are coming from today:

‍

  • Physician referrals
  • Organic social media
  • Search
  • Existing patient relationships
  • Hospital networks
  • Word of mouth

‍

Then, we assess what can realistically be improved through those channels, both through marketing and operational changes.

‍

For example, a practice may have a strong referral network but an opportunity to improve referral outreach, physician education, or scheduling processes before increasing media spend. Another practice may have an effective organic social presence that already fills schedules without paid support.

‍

Paid media should be considered when it can create incremental demand: reaching patients the practice would not otherwise reach, supporting a new provider’s ramp-up, expanding a service line, or building awareness for a procedure patients may not yet know exists.

‍

This is especially relevant when a practice introduces new technology, training, or procedures. Search can capture existing demand, but it is difficult for patients to search for a service they do not know to ask for. In those cases, paid media can build awareness around a procedure, differentiator, or physician expertise, creating demand rather than simply capturing it.

Medical Practice Growth

There is no universal answer to how much a healthcare practice should spend on paid media.

‍

The right investment should reflect:

‍

  • Current and desired utilization
  • Available clinical and operational capacity
  • The number of additional patients or procedures needed
  • Existing lead-to-patient conversion rates
  • Current cost per lead
  • Average value of a patient or procedure
  • The channels already driving demand
  • Whether paid media can produce meaningful incremental growth

‍

Your practice’s goal should be to ensure every dollar is tied to a specific, measurable business problem and has a realistic path to solving it.
